Our End of Life Carer Support Service provides emotional, practical and psychological support to carers, family and friends and to those living with life-limiting conditions in their own homes. Our aims are to:
We provide support to an individual with personal care needs following an individualised support plan and liaise with other health care professionals to ensure effective continuation of care.
Practical support to carers and their families including shopping, light domestic duties and the preparation of meals. We aim to relieve some of the pressure on carers at a very difficult time.
An important aspect of our work, our staff offer a listening ear to the person with care needs and their family and a ‘signposting’ service to other local resources to further support carers to face their caring responsibilities.
Information is the common thread linking support from one provider to another and from one ‘event’ to another. Information can be illness or person-focused. We not only focuses information on the medical condition, but also on the understanding of the patient’s preferences and values. Management is particularly important given the complexity of palliative care. We provides services that are delivered in a professional and carer-focused way.
Our services are centred around quality, predictability, consistency and flexibility for both people with care needs and their carers.
